Incident Report

Prior to submitting an incident report, members of the SODA team are available to assist with mediation if appropriate.

Depending on the nature of the complaint, updates may not be able to be given outside of confirmation of receipt. Once a form is submitted, it cannot be rescinded.

Reports related to violations of the Student Code can be submitted at the link below, or directly to the Office for Student Conflict Resolution.
Reports of harassment or assault should be directed to the Title IX Office at 844-616-7978 or titleixcoordinator@illinois.edu.
Reports regarding discrimination should be directed to the Title VI Office at 217-333-0885 or https://diversity.illinois.edu/institutional-equity/title-vi-coordinator/.
